Health Checks
If you’ve been identified as at a higher risk of developing preventable health problems such as heart disease, diabetes, kidney disease or stroke. The Health Checks team will contact you to discuss ways they can support you to make lifestyle changes to reduce that risk.
What happens after your NHS Health Check

An NHS Health Check aims to reduce the instances of cardiovascular disease (CVD) and prevent heart disease, stroke, kidney disease and diabetes and in some cases, dementia.
If you think you are eligible but have not been invited, contact your GP surgery. If you are offered an NHS Health Check, it is important that you make an appointment. Identifying risks and making lifestyle changes earlier could save your life
