Adult Wellbeing Services (AWS) is a range of services which work together to reduce health inequalities and increase opportunities for individuals and communities across County Durham.
The County Durham Approach to Wellbeing underpins underpins everything we do and is based on improving people’s wellbeing through implementing community centred approaches. The Approach to Wellbeing is about putting people at the heart of everything we do.
AWS is commissioned by County Durham Council to deliver wellbeing and lifelong learning support within the community. A new 10-year contract was awarded in late 2023 to continue to provide services which include:
Raising awareness of the signs and symptoms of cancer, the importance of the NHS Screening programmes and advising on lifestyle changes to reduce risk.
Following up those identified as at risk of preventable illness following their NHS Health Check. The service signposts clients to health and wellbeing services for further advice and support.
Provide support for communities and individuals who have previously found it difficult to access primary health care for either cultural or personal reasons.
Joining the Dots supports people aged 18 and over to live well when they have received a cancer diagnosis, are going through cancer treatment or are living beyond their diagnosis. Joining the Dots Facilitators are also there to support immediate...
A programme for patients selected from surgery or cancer treatment waiting lists who are supported whilst they wait to ensure they are physically well in readiness for the surgery or treatment.
Offering advice and training to support behaviour change and improve health, wellbeing and learning opportunities.
